• Click LOGIN under the Parent icon. 
• Select NEW ACCOUNT (or Register/New Account, depending on the screen). 
• Enter your requested info (Name, Date of Birth, Email), then click SUBMIT. (You should receive a confirmation email from FinalForms).
• Open the email from Final Forms and click CONFIRM ACCOUNT. (If you do not see the email, check the spam/junk folder). 

Log in to FinalForms

• Log in using your email and password

Log in to FinalForms

• Log in using your email and password.
• Click ADD STUDENT.
• Enter your child's legal name and required student information. 
• Click CREATE STUDENT.

IMPORTANT NOTE - STUDENT EMAIL ADDRESS REQUIREMENT
When entering your child's email address in FinalForms:
• DO NOT use your child's CSAT email address.
Students are unable to connect and sign required forms using their school email. 
• You must use a personal email address for your child so they can receive and sign required forms.

 

• From your student's account, choose the sport you are registering for (ex: Baseball, Softball, Basketball, etc.).
• Select the level: Modified (middle school level) or Junior Varsity/Varsity (high school level).

CSAT requires (per season):
• Athletic Health History Form (parent/guardian completes in FinalForms) 
• Athletic Emergency Information Card (parent/guardian completes in FinalForms) Sports Physical / Health Exam • Form (from your child’s healthcare provider—submitted per CSAT directions; can be uploaded in FinalForms). 

Make sure all required sections show as completed. Electronically sign where prompted.

Students will be ineligible to try out or practice until all FinalForms requirements are complete. – Including UPLOADED PHYISICAL WITHIN A YEAR!
