Lethality Assessment Information

ASSESSMENT PROCESS

  1. Student reports feelings of self-harm to staff member; staff member notifies school social worker.

  2. Building-level social worker interviews student and makes a determination as to whether to administer Columbia-Suicide Severity Rating Scale (C-SSRS).

  3. If C-SSRS is completed; Lethality Post-Assessment Report is completed and appropriately followed by attending social worker.

  4. Parent/guardian is contacted; Spectrum CARES (716-854-HELP) is contacted if needed.

  5. Safety Plan is completed upon the student’s return to school.
