Proofs of Residency Expired for Select Families

 

The Charter School for Applied Technologies is mandated to collect two updated (current) proofs of residency annually from each family enrolled. If you recently received a One Call Now Message from CSAT regarding your household documents on file, please note they are expired and action is required. Should your child’s file be audited; we would be out of NYS compliance.

To maintain compliancy, we are asking that families who received this call please provide two updated proofs of residency as soon as possible.

 

Documents can be submitted the following ways: 

 

1. E-mailing them back to Melinda Cebulski, chief information officer, at mcebulski@csat-k12.org


2. Faxing them to the Family Support Center at 716-871-5925


3. Taking a Photo and Texting them to Melinda Cebulski at 716-512-9608


Acceptable Documents include:

 

Primary: Mortgage Statement or Lease Agreement/ Receipt or Tax Statement/Receipt

 

Secondary: Utility (Fuel or Electric) Bill or Water Bill or Cable Bill or Phone Bill

 

PLEASE BE SURE TO HAVE THE SERVICE ADDRESS AREA (Not Mailing Address) SHOWING ON YOUR PROOFS OF RESIDENCY.
