• PHYSICALS
     
    The following students are required to have a current physical on file in the Nurses' Office:
    • All new students
    • Current students in grades K, 1, 3 5, 7, 9 and 11
      Students participating in (or trying out for) athletics **

    Students who wish to participate in interscholastic athletics must have the following items completed and on-file for EACH sports season (fall, winter, and spring):

    1. Health Exam Form - completed by the school physician or healthcare provider. It should also include the physician's name and phone number for verification. Health Forms can be faxed or uploaded (in FinalForms) Visit the forms library to upload your health forms.
    2. Athletic Health History Form -  completed and signed by parent/guardian (in FinalForms) within 30 days of the try-out dates. Grades-12 ONLY
    3. Athletic Emergency Information Card - completed and signed by parent/guardian (in FinalForms) within 30 days of the try-out dates. Grades-12 ONLY

    *To locate the Athletic Health History Form or Athletic Emergency Info Card, please log into FinalForms and click the forms tab. Once clicked, you will be prompted to complete the highlighted forms in red.     Student-athletes will be ineligible to participate in any try-outs or practices without all the aforementioned forms being completed in FinalForms. Exceptions will not be made.
