REQUIRED ENROLLMENT DOCUMENTS
The documents listed below are required for all accepted students. Parents/guardians must provide these documents when they complete the Enrollment Packet:
- The student’s Birth Certificate, Passport, or Permanent Resident Card (Green Card)
- The student’s Immunization Record
- The student’s most recent report card, progress report, or high school transcript from their current or previous school
- Two (2) current proofs of residence
Acceptable: Utility bill(s) indicating service address, lease agreement, mortgage statement, notarized landlord affidavit (form provided by CSAT), property tax bill, homeowner’s/renter’s insurance statement, DSS landlord statement
Not acceptable: Driver’s license, car insurance, credit card or medical bills, any document that does not prove physical residence
Please provide these additional documents only if they apply:
- If the student has an IEP or 504 Plan, please bring a copy
- If there are custody/guardian documents, please bring a copy
Note: CSAT is unable to accept any enrollment document submissions via fax, email, or U.S. mail from any third party.
