• UNAUTHORIZED RELEASE OF PII


    CSAT takes the privacy of its staff and student’s data very seriously and utilizes multiple security controls in order to protect this data, including following the NYS Ed Law 2d regulations which calls for third party vendors that we engage with to also provide protective security controls. In order to provide our daily education services, CSAT authorizes the appropriate use of student and staff data with the proper entities.

    However, if it is discovered that an improper unauthorized release of student PII or teacher/principal protected data has possibly occurred, then parents, eligible students (students who are at least 18 years of age or attending a postsecondary institution at any age), principals, teachers, and employees of  this educational agency may file a report about a possible incident of improper unauthorized disclosure of student data and/or protected teacher or principal data using the downloadable form below.  Please fill out the form in its entirety with as much detailed information as possible and sign and date. Once complete, please mail to: Charter School for Applied Technologies, ATTN: CSAT-DPO, 2303 Kenmore Avenue, Buffalo NY 14207.  All claims are taken seriously and will be responded to as established under NYS Ed Law 2d Part 121.
