• FORMS 

    Students who wish to participate in interscholastic athletics must have the following items completed and on-file for EACH sports season (fall, winter, and spring):

    1. Health Exam Form - completed by the school physician or healthcare provider.
    2. Athletic Health History Form - completed and signed by parent/guardian (in FinalForms) within 30 days of try out date.
    3. Athletic Emergency Information Card - completed and signed by parent/guardian (in FinalForms) within 30 days of try out date.

    *To locate the Athletic Health History Form or Athletic Emergency Info Card, please log into FinalForms and click the forms tab. Once clicked, you will be prompted to complete the highlighted forms in red. If you need help getting registered with FinalForms please click here or contact the Athletic Director, Mr. Chris Burzynski at 716-445-4496.

    Students will be ineligible to participate in any try-outs or practices without all of these forms being completed on FinalForms. Exceptions will not be made.

    SPORTS PHYSICALS

    If your child(ren) plans to participate in a modified, junior varsity, or varsity sport during the 2024-2025 school year, they will need a sports physical. Please note that the deadline to sign up for this year's in-school sports physicals has passed. You will need to obtain your own physical through your healthcare provider. Students who have not signed up will be responsible for obtaining a physical from their healthcare provider.
